*All KGV-72 turn-ins require a completed questionnaire. All KGV-72s with a "broken lock" block, also require a completed Financial Liability Investigations of Property Loss(FLIPL) IAW AR 735-5 and documentation provided with the turn-in. See memorandum COMSEC Forward Repair Activities, KGV-72 Programmable In-line Encryption Device (PIED); Direct Exchange (DX) for specific KGV-72 turn-in requirements. All other COMSEC equipment is supported by Communications SecurityLogistics Activity (CSLA) on a Case-by-Case for Turn-In Only.

MUST have proper and valid Unit Identification Code (UIC) /Army Enterprise Systems Integration Program (AESIP)/Global Combat Support System-Army (GCSS-A) CLASS VII Department of Defense Activity Address Code (DODAAC) listed and MUST match UIC / DODAAC listed on DA FORM1687, Notice of Delegation of Authority-Receipt for Supplies. If you are uncertain of your proper and valid UIC/AESIP/GCSS-ACLASS VIII DODAAC, contact your Property Book Officer (PBO).
